We designed a secure telehealth service in the I’m Ready, Test app where you can access support from health navigators before, during or after you self-test for HIV. Our health navigators can help answer your questions about the I’m Ready program, guide you to information about HIV self-testing and provide live, virtual support while you test. They can also point you to services and information for HIV care and prevention.  

Please note that our health navigators are a support and guidance service to help you find the care and information you may need, but are not regulated healthcare providers and cannot provide medical care. If you need help for mental or physical health needs you should seek medical care.  

Who are our health navigators?

Our health navigators are a diverse and dedicated team from different regions in Canada who are committed to increasing access to HIV testing, care and prevention. They aim to provide support that is culturally safer, anti-racist, affirming of sexual orientations and gender identities, sex-positive, respectful and judgment free.  

Each health navigator brings their own lived experience to the table and has been trained specifically for this program to guide you to the right information on HIV care and prevention for where you’re at.  


How to connect with a health navigator through I’m Ready, Talk  

  1. Once you’re part of the I’m Ready program and have created a profile through the I’m Ready, Test app, you can access the secure telehealth service by clicking on the Connect tab within the app.
  1. Pick your health navigator and appointment time    
     
    From the Connect tab on the I’m Ready, Test app, click on the “Talk to a Health Navigator” button, and then the “See next available appointment times” button. From there, you’ll be able to see brief bios for each health navigator who is available. You can select your preferred health navigator and select the time that works best for you.
  1. Choose your appointment type

    After you select the health navigator you want to meet with and the preferred appointment time, you can choose if you want to attend an audio, video, or chat appointment. You also have the opportunity to leave a note for the health navigator in the event you have something specific you would like to talk to about. Click on the “Book appointment” button and your request will be sent to the health navigator to confirm.

    Please note that the I’m Ready, Test telehealth service is not an on-demand service and is not available 24/7. Health navigators are on-call and may not be available at your requested appointment time. If a health navigator is not available at your preferred time, you will receive a notification inviting you to book for another time. We offer appointments Monday-Friday, from 10:00am to 10:00pm ET. We are closed on statutory holidays.

More about the I’m Ready, Test service 

The REACH Nexus team, part of MAP Centre for Urban Health Solutions at St. Michael’s Hospital, Unity Health Toronto, worked closely with our partners, the Community-Based Research Centre (CBRC) and Women’s Health in Women’s Hands (WHIWH), to design the I’m Ready, Test health navigation telehealth service.
